*Print the form at https://www.cumberlandvalley.coop/kentucky-sales-tax-utilities and mail (PO Box 440, Gray, KY 40734), fax (606-526-0158), email (tax@cumberlandvalley.coop) or bring it to our office.

*Come into the office and fill out the form in person.
