Beginning January 1, 2023 only a person’s primary residence (domicile) will be exempt from sales tax on utility services. *Print the form from our website and mail (PO Box 440, Gray, KY 40734), fax (606-526-0158), email (tax@cumberlandvalley.coop) or bring it to our office.

*Come into the office and fill out the form in person.
